An Advance Healthcare Directive, also known as living will, personal directive, advance directive, medical directive or advance decision, is a legal document in which a person specifies what actions should be taken for their health if they are no longer able to make decisions for themselves because of illness or incapacity. A POLST Form (Provider Orders for Life-Sustaining Treatment) is an approach to improving end-of-life care in the United States, encouraging providers to speak with patients and create specific medical orders to be honored by health care workers during a medical crisis. It is important to note that in order for a DNR to be honored outside of the inpatient setting (in the community) you must have a POLST Form.
